Steps to Health Coordinator - Hengrove Park Leisure Centre - Bristol

This is a role where you will get a chance to develop and coordinate a local Exercise Referral Program which includes delivery of exercise opportunities for clients with chronic conditions to standard protocols to ensure an appropriate, equitable and accessible service across the Local Authority

Primary Objectives

  • To ensure the safety and enjoyment of everybody using the GP referral programme
  • To promote the programme with local health professionals and Parkwood Healthcare
  • Maintain partnerships with key agencies, health professionals, communities and organisations to establish and provide structured patient exercise and education programmes. When needed, develop new partnerships
  • Maintain high levels of uptake, encourage new ideas to increase participation and retention to physical activity via the scheme
  • To analyse statistics from the GP referral programme in order to evaluate the success of the scheme

The Person

  • Must be able to gain acceptance and respect from clients and the teams involved.
  • Must be able to build strong relationships with customers, clients and employees.
  • Must be able to communicate at different levels of the organisation, both listen and direct.
  • Must be able to demonstrate clear identification with all customer requirements.
  • We are looking for candidates who hold a recognised Level 3 qualification in exercise referral and have experience in delivering prescribed exercise to referred patients. Although not essential experience in building relationships with community and clinical partners to create referral pathways into the service would be desirable.
