Parkwood Healthcare Ltd are recruiting for a part time permanent Live Well Stay Well Coach to join our team in Buckinghamshire (20 hours per week).

Live Well Stay Well is an integrated lifestyle service provided by Parkwood Healthcare and commissioned by Buckinghamshire Council.  The service, delivered across the County, delivers Healthy Lifestyle support to Buckinghamshire residents. 

The Opportunity

The role of Live Well Stay Well Coach involves:

• Using our Get Healthy software to assess our clients lifestyle risk factors
• Using a combination of motivational interviewing, goal setting and behaviour change techniques to support our clients to make long term changes to their lifestyles
• Working with a specialist team to provide advice on weight management, smoking cessation, physical activity and NHS Health Checks
• Putting the client at the centre of everything we do
• Working with a range of other specialist healthy lifestyle services across Buckinghamshire
• Keeping in touch with our clients by phone, email and text message on a regular basis
• The working hours will be between the hours of 8am-8pm Monday to Friday and 9am - 4pm on Saturdays, on a rota basis

Full training will be given to the successful applicant but previous experience of working in a health improvement environment will be an advantage.
