Fixed Term Contract - 12 months

The Belfast Healthwise team are looking for a Health Referral Facilitator to work at Olympia Leisure Centre in Belfast, Northern Ireland.

An individual who has a genuine passion for exercise referral and wanting to positively impact on people's lives for the better through physical activity and lifestyle. The role is diverse and challenging with a range of requirements that focuses on interventions that help the management of existing medical conditions and/or act as preventative measure to reduce risk factors for chronic conditions or comorbidities.

These requirements include;

  • Delivery of Tier 2 adult weight management programmes (10 weeks) at various locations.
  • Delivery of physical activity on referral (12 weeks) at various leisure centres.
  • Facilitating supervised gym sessions and classes for Healthwise members
  • Working with a range of medical conditions with an emphasis on obesity, diabetes, mental health, Cancer and falls prevention.
  • Support health promotion, awareness and outreach with health professionals, affiliated organisations and community groups to make the borough of Barnet more active and healthier.

You may be required to work unsociable hours, which include mornings, evenings, weekends and bank holidays to meet the needs of the service.

If you're a fitness professional or someone with an academic background in a sport or health-related discipline and hold relevant vocational experience that's confident, self motivated, empathetic and able to inspire a wide range of people then look no further.

This means you'll be required to have the following;

  • REPS registered with a Level 3 Exercise referral qualification (essential)
  • Experience with special populations (Elderly, Diabetes, Cancer, Falls prevention and mental health)
  • Level 3 Nutrition/weight management qualification
