We are recruiting a Wellbeing Advisor to help deliver our exciting Wellbeing Prescription (Social Prescribing) service which works to promote lifestyle change and prevent ill-health.

As a Wellbeing Advisor you will work 1-2-1 with clients in GP surgeries and/or with clients in their own homes.

Using behaviour change techniques you help to identify their health and social needs, encourage change and signpost them to services that can help them lead a healthier lifestyle.

Clients may be referred to you for a variety of reasons including loneliness, anxiety, obesity and inactive lifestyle. Using appropriate techniques, you will support the client to change their behaviour and improve their wellbeing.

You will work within East Surrey GP surgeries which cover Reigate and Banstead districts and Tandridge districts.

This is an exciting opportunity to be part of a successful, expanding Social Prescribing service. The post is initially funded until March 2021, with the possibility of continuation subject to funding.

Interviews are scheduled to take place on the 4^th March 2019.
