Physical Activity Coordinator - Sheffield Contract

Temporary Contract until 31st December 2019 (potential for further extension)

 40 hours per week - requires flexibility to ensure we meet the needs of the service and may include evenings, weekends and bank holidays.

The post is subject to a full DBS disclosure.

An exciting opportunity has arisen to join our dynamic team of health and exercise professionals in Sheffield, who deliver a range of services as part of the physical activity programmes offered across the city. We are looking for an experienced, enthusiastic and dynamic Physical Activity Referral Specialist who will manage and add value to the programmes that we deliver, which support patients who are referred via GP or health care professionals for a variety of medical conditions.  

As part of the team you will be responsible for coordinating, delivering and promoting the programme and are required to complete one to one assessments with patients and lead and deliver exercise programmes and classes. The successful candidate must be an excellent communicator, with a high level of organisational skills, have experience of exercise referral work and hold the necessary professional and statutory qualifications, including a Level 3 Exercise Referral and Level 4 BACPR Cardiac Rehabilitation qualification. It would be desirable to also hold Level 4 Specialist population CPD’s (such as cancer, lower back, mental health and/or pulmonary rehabilitation) along with nutrition and public health related awards.

Role and Person Specification:

  1. Experience of working with individuals with medical conditions in an exercise related setting.
  2. Experience of delivering a range of specific 1 to 1 exercise/physical activity sessions including gym based exercise and health classes.
  3. Ability to conduct individual motivational interview consultations and assessments for patients on a one to one basis.
  4. Ability to collect, collate and evaluate data in order to monitor the success of the programme.
  5. Ability to accurately enter and maintain participant’s data records using a range of database sources following data protections laws.
  6. Excellent time management and organisation skills with the ability to prioritise work areas and meet deadlines.
  7. Demonstrate an empathetic nature, along with having a positive and motivated approach to work.
  8. Excellent communication skills and professional telephone manner.
  9. Well developed IT skills, especially in Outlook and Microsoft programmes
  10. A professional manner and strong interpersonal skills with the ability to work closely with health professionals, nurses, social services and other regulatory bodies to receive and arrange referrals.
