Community Navigator​ - Luton

Full time – 37 hrs per week (occasional evenings and weekends)

Social Prescription (SP) is a new way of linking patients with sources of support. The Community Navigator will work closely with GPs to support individuals to access and connect with appropriate services and activities within the Community, Voluntary and Statutory sectors.  The focus of the service is on prevention and early intervention to empower people to take control of their health and wellbeing.

This post will be based predominantly within one GP cluster, working in GP practices serving key neighbourhoods.

The successful candidate will contribute to the development of Active Luton’s community wellbeing strategy through the signposting of patients and supporting the development of additional programmes that meet the needs of the community.
